Need to re-order your pet’s medication?

Please use this form to request a repeat prescription for your pet’s medication. We will let you know when it’s ready for collection so you can drop in to pay & collect at a time that suits your schedule.

Medications that are in stock are usually ready the same day. If they are not in stock, then please allow 3 business days for the medication to be ready for collection. If urgent, please contact the clinic directly over the phone instead of using this form.

***Some medications are not able to be dispensed without a consultation.***

Please include in the below form :

• Name of medication
• Number of tablets/boxes etc requested
• Concentration of the medication

For example - Pimobendan tablets, 1 box, 2.5mg
