Need to re-order your pet's medication?
Use this form to request a repeat prescription. We'll let you know when it's ready for collection so you can drop in to pay and collect at a time that suits you.
Medications that are in stock are usually ready the same day.
If not in stock, please allow 3 business days for the medication to be ready for collection.
If urgent, please contact the clinic directly over the phone instead of using this form.
Some medications cannot be dispensed without a consultation.
Please include in your request
- ·Name of medication
- ·Number of tablets, boxes, etc. requested
- ·Concentration of the medication
- ·For example: Pimobendan tablets, 1 box, 2.5mg
